We know hospitals, while necessary for immediate and acute care, are not great places for our elderly population to stay in for too long. CHAW Care Community are working on several resources to help change this:
- Developing an urgent response team to help residents stay out of Hospital
This multidisciplinary team works with primary care and 999 to provide prompt and at home medical assistance, reducing the need for A&E attendance.
- Enhanced support to Care homes using Feebris. A remote monitoring device that helps carers make more accurate medical assessments so the seek appropriate advice sooner
